INSTRUCTIONS FOR S.M.K. APPLICATION

Print out this application, fill out as indicated, Sign and have notarized. Include copies of all certificates from Kyu level (Shodan and above are required). Example, if you claim to hold the grade of 7^th dan or equiv. you must submit 1^st through 7^th dan. If you had skipped a grade we will need some form of documentation for same. ( The certificates up to fifth dan or equiv. Must be from the same ryu / ryu – ha.

Note: Copies of certification from Shodan and above are only required for your main (the one art / way) art.

If you were ever convicted of a crime, unless you feel it was minor , please do not apply. If the latter, please submit in writing why you feel it should not be held against you.

Also enclose with the application the following.

Any document you may have demonstrating you have a clean (criminal free ) background.

A copy of your written grade requirements up to and including fifth dan or equiv.

On a blank paper, apply in ink, your signature seals ect that are used to seal your certificates. Also translate their meaning in writing under said seal .

A Copy of your ( blank) certificates (one of each type you issue) with VOID written across the entire certificate.

Send with a check / money order made out to Donald P. Jeffrey for 20.00 application fee.

(If approved, the 20.00 fee will be deducted from your membership fee).

Please fill out your drivers license information. We require it for a background check.

Attach a 1,000 word thesis on your art / way. It's concepts, theories, philosophies and history as well as all arts that influenced your art.

Professional Standards Agreement

I hereby agree to the Soke Menkyo Kai Professional Standards Agreement as posted on their web page (www.sokeship.com) . I further realize that if it is discovered that any false information is given / submitted by me to the Soke Menkyo Kai for the purpose of obtaining membership / accreditation will result in anything issued to me by the Soke Menkyo Kai, including all grades, ranks, titles and / or accreditation being revoked and I agree to return any all certificates , licenses and / or titles to the Soke Menkyo Kai without any recourse for any form of refunds, i.e.; membership fees ect.

I further agree that if I break / disobey any of the Professional standards Agreements, I will resign as in accordance with above.

I also agree that if it is found that any individual under my control (Sokeship / martial arts organization) does not also follow the Professional Standards Agreement I will take corrective measures which may include revocation of the individual from my organization / Sokeship and I further realize this may also come to my revocation of membership with the Soke Menkyo Kai.
